with COVID CARD and BLS. JOB DETAILS :
Our client, located in Canton, Massachusetts, is a state-operated
pediatric chronic care hospital under the Department of Public
Health. It provides comprehensive medical, rehabilitative,
educational, and recreational services to children and young adults
(ages 7-22 ) with multiple disabilities, helping them achieve
greater independence. Services include 24/7 nursing care, various
therapies, special education, adaptive equipment support, and a
range of therapeutic and recreational programs. The Case Manager
coordinates, monitors, develops and implements comprehensive
treatment plans for developmentally disabled patients. The Case
Manager reviews treatment outcomes and provides communication with
state and local agencies to ensure program implementation. Makes
decisions through problem solving, instruction of other employees,
negotiation and reconciliation of differences with members of the
treatment team. Performs other work-related duties as assigned by
supervisor.
- Leads interdisciplinary treatment team in discharge planning
process with knowledge and experience with discharges from hospital
environments related to youth transitions to adulthood.
- Interview patients and families at a time of pre-admission
evaluation to assess appropriateness for admission. Document
findings and make recommendations to Admission Committee.
- Meets with families of each new patient, obtain a complete
psycho-social history which is placed in the patient's medical
record.
- Reviews and analyzes data on a continuous basis through the
care planning process to determine progress by measuring outcomes
to make recommendations for changes in procedures affecting
guidelines, interventions and approaches that accomplish
program/treatment objectives.
- Evaluates and analyzes information about patient in order to
suggest approaches for resolving legal, family, social, economic,
behavioral and/or physical problems. Assists with solving problems,
to determine patient's progress and to inform patients about their
eligibility status etc.
- Able to demonstrate knowledge and skills regarding age specific
physiological changes, developmental needs, equipment needs and
appropriate therapeutic interventions.
- Knowledge of principles of growth and development over the life
span and interpret the appropriate information needed to identify
each patients requirements related to age specific needs.
- Assist patients and families in discharge planning for patient
in order to assume a smooth transition in the community upon
discharge.
- Explore housing, education and medical options in the community
with the patient and family in order to provide best possible plan
for patient upon discharge.
- Participate in patient/family/education in the areas of
treatment planning, process and progress, adult service providers
and insurance information.
- Facilitate regular unit meetings designed to educate patients
in financial, community, organization and social skills to achieve
highest level of independence in each case.
- Identifies opportunities in improving service delivery and
participates in developing new processes/solutions.
- Participates in data collection.
- Demonstrates critical thinking and sound judgement in clinical
and administrative decision making. QUALIFICATIONS REQUIRED AT HIRE

- Ability to interact between MA Agencies with varying
perspectives, such as DDS, DCF, DPH, DESE and local school
districts.

MINIMUM ENTRANCE REQUIREMENTS:
Applicant must have at least three years of full-time or equivalent
part-time, professional experience as a licensed social worker or
active certification as a child protective worker as permitted by
state law.
